Barinov Y. Optimization of surgical repairing of upper lacrimal system draining disorders

The dissertation is dedicated to increasing effectiveness of surgical treatment of patients with stenosis and obliteration of upper lacrimal system which is based on using a bundle of silicone threads and gentle way of their introduction into nasolacrimal duct, and lacoprosthesis made of titanium alloy of original construction. Based on complex examination of patients with tear-draining disorders criteria for choosing a method of surgical treatment of stenosis and obliteration of upper lacrimal system were specified. In order to restore tear drainage in case of strictures of upper part of lacrimal duct aiming temporary intubation, original secure method of introducing a silicone threads bundle with the help of magnet and conductor with a magnet ball on the end was applied. Lacoprosthesis of titanium alloy of original construction for permanent intubation in case of complete obliteration of lacrimal ducts has been designed. The way of implantation of artificial lacoprosthesis in case of total obliteration of upper lacrimal system has been improved. Reaction of conjunctiva epithelial cells which surround distant part of lacoprosthesis has been examined and importance of using titanium alloy for producing an artificial lacoprosthesis has been proved. We've shown effectiveness of original method of lacoprosthesing with prosthesis made of titanium alloy of original construction in treatment of obliterations localized in upper lacrimal system, which gives an opportunity to restore tear draining in 92 % prosthesing cases.
