Dissertation is devoted to questions of increase in treatment efficiency of duodenal ulcer associated with recurrent H.pylori, connected with chronic pyelonephritis on basis of study of clinical course peculiarities. For estimation of current disease severity and adjustment of adequate antibacterial therapy we showed expediency of detection of peculiarities of duodenal ulcer clinical manifestation associated with chronic pyelonehritis in clinical practice. Multidirectional deviations in physiological norms of levels and processes of formation, imbibition and utilization of short-chain fatty acids at associating of duodenal ulcer with chronic pyelonephritis may serve as biochemical markers of structural and functional disorders of intestinal microbiocenosis at clinical estimation of patient's state. We developed rational approach to complex therapy of duodenal ulcer associated with chronic pyelonephritis with the use of combination of rabeprazole, claritromicine, amoxiciline/clavulanate, lactovite and canephrone H, and as a result we succeeded in approaching of considerably greater duration of remission and stability of treatment results of associated diseases.
