Object of the study: glomerulonephritis (GN) in children. Purpose of the study: a higher efficacy of treating acute glomeru-lonephritis (AGN) in children on the basis of the study of the role of cytokines in the manifestation and progress of the disease. Methods of the study: general clinical, biochemical, immunologi-cal, immunoenzyme. Practical significance: an importance of the study of cytokines (interleukin-1? [IL-1?], interleukin-4 [IL-4] and tumour necrosis factor alpha [TNF-?] for assessing the severity and clinical activity of GN in children is demonstrated. A way for prognosticating the risk of AGN chronization in children is suggested. With regard for a cyclic character of the synthesis of proinflammatory cytokines during 24 hours, a scheme of administration of disaggregates in compliance with circadian changes of IL-1? levels is scientifically grounded and suggested. Novelty of the obtained results: a higher activity of proinflamma-tory cytokines with different GN syndromes is revealed. It is shown that in the period of initial manifestations in AGN there is an in-crease in blood levels of IL-1? and TNF-?. A severe course of AGN is characterized by a steady increase of IL-1? blood levels, this phenomenon being preserved even in case of normalization of clinical-laboratory data. A prognostic significance of IL-1? as a marker of the risk of GN progression, whose scientific novelty is confirmed by a state patent on invention No. 70904 A dated Octo-ber 15, 2004. It is for the first time that some fluctuation in cyto-kine levels during 24 hours in the blood plasma of children ill with GN. It is shown that the maximum of IL-1? secretion occurs in the evening time. The influence of drug preparations, used for treat-ment of GN, on the secretion of cytokines is found out. A scheme is suggested for administering disaggregates with properties of the influence on proinflammatory cytokines with regard for the re-vealed changes.
