Essay

Thesis is devoted to the actual questions related to diagnostics of thoracic aortic aneurysm for the optimum choice of the operative treatment and subsequent evaluation of its efficiency on the basis of complex echocardiography. The anatomic-and haemodynamic features of dissecting thoracic aortic aneurysm were invastigated before and after operations on the basis of complex echocardiography, that consisted of transthoracic B-mode echocardiography, continuous-wave and impulsive Doppler and colour Doppler, esophageal echocardiography. The research is based on the investigation of 102 dissecting thoracic aortic aneurysm patients. The anamnesis data, EKG, were registered, X-ray of the thorax; the complex echocardiography was conducted at the inspection of patients. Invasive methods of diagnostics - cardiac angiography was executed on 100 (98,0%) patients with dissecting thoracic aortic aneurysm. The operations with the use of artificial circulation were done on 76 patients.The sensitiveness of the esophageal echocardiography at the proximal dissection was 95,8% and 100% at distal dissection. The diagnostic value of esophageal echocardiography prevailed above transthoracic for most direct and indirect signs of dissection. In the research work there was generalized the experience of complex echocardiography usage for the dissecting thoracic aortic aneurysm diagnostics; the early dissecting thoracic aortic aneurysm diagnostics protocol of method was specified.
