Essay

In dissertation, on the base of complex clinically-laboratory, instrumental and radioisotope investigation of 120 patients with essential hypertension (EH), chronic obstructive bronchitis (COLD) and in their combination, the generalized data is given in relation to the role of endothelial dysfunction in the pathogenic mechanisms of left ventricle (LV) remodeling. Direct correlation dependence is set between the degrees of pulmonary insufficiency, EH and by the basal level of some pressor neurohumoral substances (adrenalin, noradrenalin, renin, aldosteron) and development of LV dysfunction (forming of eccentric hypertrophy and concentric remodelling) for patients with EH in connection with COLD. The role of cortisol is certain, as a marker of bronchial obstruction. Efficiency of combined antihypertensive therapy is proved for angiotensin converting enzyme inhibitor enalapril maleas and calcium channel blocker dilthiazem (diacordin) in relation to the improvement of the of endothelial function state, cardiohemodynamic, to regress of hypertrophy at patients with EH in combination with COLD.
