Essay

Object: equinoplanovalgus foot deformity in spastic cerebral palsy. The aim: to improve the efficacy of surgical and conservative methods of treatment in patients with spastic equinoplanovalgus foot deformity. Methods: clinical,X-ray examination, biomechanical, statistical. Scientific achievments: a new knowledge of clinical and functional foot changes in spastic has been gained. It has been revealed that expressed equinoplanovalgus foot deformity is combined with less calcaneous pronation and less decrease of longitudinal arch, and after operetive correction of equines the planovalgus deformity progresses. New X-ray methods have been invented to carry out more detailed examination of foot longitudinal arch condition. A methodic of orthopedic treatment of spastic equinoplanovalgus foot deformity has been elaborated which concludes application of BTX-A and further rehabilitation. A new methodic of bone graft calcaneotomy in children with equinoplanovalgus foot deformity 8-12 yeras old has been improved.It includes using of cuneiform bone graft which gives possibility to correct calcaneous pronation and to increase longitudinal arch height. A methodis of tripple arthrodesis of the foot has been elaborated that includes additional approach to talo-navicular joint and lengthening of lateral foot column. Aprobation of above-mentioned methodics has proved their efficacy: improved methodic of calcaneous bone-graft osteotomy has achieved 64,3% of good and 35,7% - satisfactory results; tripple arthrodesis after our methodic has achieved 66,6% good and 33,4% satisfactory results. The methodics have been included in practics and educational process of department of orthopedic and traumatology in Odessa state medical university, in Odessa city hospital №11, in Odessa rehabilitation Center for disabled children, in specialized clinical sanatorium for children "Hadujbey".
