Object of the study: intestinal rotaviral mono- and mixed infection. Purpose of the research: improvement of early diagnosis and search for ways to increase the efficacy of therapy for intestinal rotaviral-bacterial infection in young children on the basis of studying its clinical picture, peculiarities in the blood interleukin reaction and the patients’ immune response. Methods of the research: clinical, bacteriological, serological, immunological and statistical ones. Theoretical and practical results: results of the research broaden the range of knowledge of practitioners about the pathogenesis of intestinal infections of the rotaviral and rotaviral-bacterial etiology in young children, thereby making it possible to objectively assess the pathological process severity, determine the scope of therapeutic interventions on good grounds and choose directions for improving the therapy for the patients and rehabilitations for the convalescents. On the basis of the revealed informative anamnestic, clinical-laboratory and immunological indices, a method for diagnosing intestinal infections of the rotaviral and rotaviral-bacterial etiology was developed and suggested for introduction into the practical activity of health care institutions. Its use makes it possible, even at early stages of the pathological process manifestation, to diagnose the mono- and combined etiology of the disease, to correctly and timely determine the scope for the pharmacological intervention, and to lay down ways for improving the efficacy of the patients’ treatment. Practical health care institutions were suggested diagnostic criteria for assessing the completeness of the patients’ recovery, and they can be an argument in the choice of rehabilitation measures at the stage of dispensary observation of the convalescents. Novelty: it was found out that in recent years the clinical picture of rotaviral lesions of the gastrointestinal tract in young children had significantly changed, thereby causing the appearance of the topical variants of the pathological process, which were not characteristic of the above infection before. Some clinical peculiarities of the rotaviral-bacterial intestinal infection were revealed, and on their basis a differential-diagnostic table was made, whose use at the early stages of the pathological process manifestation makes it possible to diagnose the mono- or combined pathology of the alimentary canal in children with a high accuracy. It was proved that immune parameters were of a high diagnostic significance and could serve as the basis for developing new diagnostic and differential-diagnostic ways. A method for diagnosing the rotaviral mono- and mixed infection was developed, and it was recognized as an invention (patent for a useful model “A way of diagnosing the rotaviral mono- and mixed infection” No. 24023 dated June 11, 2007, bul. No. 8). Probable ways for increasing the efficacy of the treatment of patients with the intestinal rotaviral infection were theoretically grounded. Degree of introduction: resultsof the thesis were introduced into medical practice of Regional Children’s Infectious Hospitals of such cities as Kharkiv, Vinnitsa, Zaporizh’a, Lviv, Ternopil, Donetsk, Dnipropetrovsk City Hospital No. 21; educational process of the infectious type departments of Kharkiv National Medical University, Kharkiv Medical Academy of Postgraduate Education, Vinnitsa National Medical University, Zaporizh’a State Medical University, Dnipropetrovsk State Medical Academy, Donetsk National medical University, Lviv National Medical University, Ternopil State Medical University. Sphere of application: medicine, paediatrics.
