The thesis is devoted to the problem of raising the efficiency of the treatment of patients suffering from inflammatory diseases of festering maxillary sinuses (such as acute festering maxillary sinusitis and acute attack of chronic festering maxillary sinusitis) by means of the method of photodynamic therapy. The proposed method is substantiated experimentally and clinically. Experimental investigation of the optical absorption peaks of antiseptic dyes applied in medicine, was carried out. The light emission sources were chosen in such a way that their use, in combination with the action of the dyes, exerted bactericidal effect on staphylococcus aureus due to photodynamic reaction. The said effect was not obtained at individual action of dyes or light emission on the microorganism. In the performed experiments minimally active dye concentrations were selected, and their safe effect on the mucous membrane of maxillary sinuses was proved. The highest antibacterial effect was obtained for brilliant green dye combined with radiation of light of the red region of the spectrum. Examination and treatment of patients suffering from festering inflammation of maxillary sinuses was realized. The comprehensive study included endoscopic, cytological, bacterial and X-ray examination methods, as well as laboratory investigations. The sanative activity of the method of photodynamic therapy was found to exceed that of the traditional method of therapy based on the use of antibiotics and infrared radiation. Clinical efficiency of the proposed method was demonstrated. The advantages of the method of photodynamic therapy are its universality, reasonable cost, effectiveness even in the cases when antibiotic-resistant strains of causative agents are present, simplicity of realization.
