Rudenko S. Left ventricular aneurysms of the ischemic genesis: diagnostics and treatment

Dissertation is dedicated to study the problems of left ventricular aneurysms (LVA) of the ischemic genesis diagnostics and surgical treatment, to elaboration, argumentation and the use of the most effective methods of left ventricular plasty and protection. Dissertation material consisted of 789 patients, operated on because of the LVA. Diagnostic ralue and trustworthness of different methods of diagnostics were analysed. Elaborated methods of left ventricle protection permitted to decrease quantity of intraoperative myocardial infarctions. Indications to the use of a different methods of left ventricular plasty in dependence of LVA localization and extension were grounded. As the result of the use of new methods of myocardial protection together with a differential approach to the methods of left ventricular plasty was the decrease of postoperative mortality from 10,4% to 2,7%. Analysis of the main postoperative complications in LVA surgical treatment and evaluation of immediate and of remote postoperative period permitted to elaborate measures of complications prevention and treatment to determine factors, wich negatively influence on the operative results and to offer methods of their prophylaxy.
