Essay

Dissertation is devoted to the problem of surgical treatment results improvement at patients after gunshot and explosive injuries of vertebral column and spinal cord in the late period. The research is based on the analysis of 120 clinical observations of patients with spine traumatic disease caused by gunshot and explosive injuries gunshot and explosive injuries, which were treated in Saki specialized spinal sanatorium named after acad. M.M. Burdenko and in the clinic of renewal neurosurgery of Saki central military sanatorium named after M.I. Pirogov in 2000-2007. The novelty of the research lies in working out of indications for differential surgical treatment at patients with gunshot and explosive injuries of vertebral column and spinal cord in the intermediate and late periods in the suggested complex of neurotrophic complications treatment. According to the obtained data, improvement of neurological symptomatic was observed in 35,4% cases after operations on vertebral column and spinal cord, at conservative treatment only - at 29,1% patients. On the whole, the renewing treatment of 120 patients with consequences and complications of gunshot and explosive injuries of vertebral column and spinal cord, has given positive result in 30% cases. Better results of spinal cord functions renewing were observed at patients with non-penetrating injuries of vertebral column: at the cervical part of spinal cord - in 2 (66,6%) cases, thoracic - in 3 (20%) and sacro-lumbar - in 3 (99,9%). Reconstructive-renewing operations, performed on vertebral column and spinal cord in the intermediate and late periods are effective at progressive instability of vertebral column, pain syndromes and myelopathies, caused by compression.
