Dissertation is devoted to study of peculiarities of hemodynamic disorders, vasoregulating function of vascular endothelium in patients with chronic obstructive pulmonary diseases II and III stage combined with arterial hypertension II stage and impact of monotherapy with losartan kalium on the processes studied.There were established peculiarities and stages of disorders of diastolic function of cardiac ventricles and daily profile of arterial pressure. There was assessed perspective of improvement of cardiohemodynamics and function of vascular endothelium by means of angiotensin II antagonists in patients with COPD and with concomitant essential arterial hypertension. It was proved that a prolonged usage of losartan kalium in a daily dose of 50-100 mg favors normalization of diastolic function of cardiac ventricles, increase of nitrate nitrogen concentration in blood serum and rise of endothelium-dependent vasodilatation; this may be considered as improvement of vasoregulating function of vascular endothelium. To increase treatment efficacy in patients with COPD with concomitant arterial hypertension, representative of a group of angiotensin II antagonists -losartan kalium may be administered as a drug of choice into the composition of a complex therapy for improvement of vasoregulating function of vascular endothelium.
