Mіronenko N. Surgical Restoration of the Voice after a Laryngectomy

The dissertation is devoted to the decision of an important task, to the improvement of the quality of life of patients with the cancer of larynx after the laryngectomy by the recreation of voice with the simultaneous forming of dividing function of shunt and account of the differentiated testimonies operations of the voice restoration. The basis of the work is made by the experience of surgical treatment of 123 patients with the cancer of larynx in the III and the IV stages diseases. The developed method of the voice restoration after a laryngectomy is analysed, at which the damage of muscl?s layer of gullet diminishes during the operation, wich leads to the improvement of protective function of the bypass opening. On the basis of comparative analysis of the phonation and protective functions of shunt efficiency of the offered method is well-proven in the inspected groups of patients. The bioelectric activity of muscles of neck department of gullet is first studied in the thesis at patients after a laryngectomy with the different types of the tracheoesophageal shunts and the dependence between the protective function of shunt and bioelectric activity of muscles of neck department of gullet is set by. It is first well-proven that row of parameters of electroneuromiography muscles of neck department of gullet (amplitude and duration of potential of motive unit) can serve as the criteria at the choice of optimum method of restoration of the voice of patients after a laryngectomy.
