The work covers the questions of improving radiation methods of investigation of the patients with differentiated thyroid carcinoma (DTC) with iodine negative metastases and/or relapses and determining pathogenetic ways of radioiodine resistance overcoming.Based on the obtained findings, frequency of iodine-negative metastases and relapses in patients with differentiated thyroid carcinoma with iodine negative scans was determined. The role of thyroglobulin investigation in forming risk groups of iodine negative metastases and relapses of thyroid carcinoma was studied. The indications to and diagnostic significance of scintigraphy with 99mTc-pertechnetate, 99mTc-MIBI та 99mTc-(V)DMSA in visualization of iodine negative metastases and relapses of thyroid cancer were determined. Pathogenetic causes of thyroid radioiodine resistance in patients with iodine negative metastases and relapses of differentiated thyroid cancer were studied. Target methods of overcoming radioiodine resistance by use of drugs increasing the degree of the tumor tissue readiness to absorb radioiodine via stimulation of circulation, redifferentiation of the tumor metastatic tissue and increase of endogenous TTH were suggested.For the first time, the classification of iodine-negative metastases and/or relapses of thyroid cancer was suggested. The term «iodine-negative thyroid tumor» was defined.
