Essay

The subject is the efficiency of palliative combined and complex treatment of initial non-organic malignant neoplasms(INOMN) of retroperitoneal space and peritoneum. The purpose is to increase treatment efficiency of patients with INOMN of retroperitoneal space and peritoneum (ICD-10,heading C48) by means of optimizing and applying new methods of combined and complex treatment.The approaches are clinical, instrumental, radiological, CT, MRT, and morphological, statistical.The novelty is that for the first time the efficiency of endolymphatic chemotherapy for relapses have been studied and proved.By means of statistical analysis it was found that the combined treatment both of primary and recurrent tumor,including radiotherapy, endolymphatic chemotherapy,allows improving follow-up results of treatment.There have been developed indications for various types of special treatment depending on prognostic factors. The results present justification of combined treatment efficiency:occurrence of relapses in the first two years after surgery becomes less by 2.3 times(95%,confidence interval of 1.1-4.9) compared with surgery.The expediency of tumor partial excision has been proved. There was established expediency of applying conservative special treatment approaches. The developed ways of treatment have been implemented into practice at the Donetsk Regional Anti-Tumor Center,Mariupol City Inter-regional Oncology Dispenser,Municipal Oncology Dispensers of Gorlivka and Kramatorsk.The work results are used in training by the Oncology Departments of the М.Gorky Donetsk National Medical University.
