Methods: information- patent research; bio-mechanical research of the pivot external pawls fixing qualities; analysis of the tense- deformed conditions of the lower one third of leg and foot fixed by pivot external pawls using the method of terminus elements; x-ray patients examination in a treatment dynamics; clinical patients examination in treatment dynamics; comparative analysis of the clinical materials. As a result of the research it was reviewed the methodical principals of trauma heaviness estimation and trauma illness prognosis in open damages of the ankle joint, which helped to define new views on treatment tactics: the negative results were stipulated by the heavy necrotic-suppurative complications; in a basis of prophylaxis of the suppurative complications of the open damages of ankle joint according to the surgeon and medicament methods there is a reasonable choice treatment immobilization means. Biomechanical research of the characteristics of fixation pivot external pawls let substantiate its' choice for open ankle joint damages immobilization by the criteria of fixation toughness; by the method of the terminus elements it was based adequacy of its' using on the "ankle-foot" segment. Optimization of surgeon and medicament prophylaxis means of the inflectional complications in the open damages of ankle joint according to the rational ways of the treatment immobilization, the tactic of a patient's after-operational control are the components of the open damages of the ankle joint treatment, it has given good improvements to the results of treatment and reduce the quantity of the heavy invalidizative complications in the main supervised group comparing to the control one on 29, 5 % . The results of the work were inculcated in a practice of help rendering to the patients with open damages of the ankle joint in a city multy-specialized hospital
