Yakovenchuk N. Diagnostic and therapeutic blocks in the case of hip-spine syndrome.

Objective researches: a painful syndrome of with coxofemoral and lumbar localisation. Research objective: to prove and develop methodology of use of medical-diagnostic blockade for increase of efficiency of therapy of patients with a hip-spine syndrome. Research methods: clinical investigations (orthopedic, neurologic, muscle testing, diagnostic blocks) have been applied, instrument methods of investigation (X-ray examination, computerized tomography, doppler ultrasonography, renovasography, thermography) and topographic-anatomical investigations have been used. Figures it is processed statistically. Factors of occurrence and variability of clinical displays of a painful syndrome are established at HSS. On the basis of use of medical-diagnostic blockade, new knowledge of mechanisms of realisation of a painful syndrome are received at a hip-spine syndrome and participation of various anatomic formations in formation of the local and reflected pain is defined. For the first time dominating participation vertebral and extraarticulate factors in formation of a pain of with coxofemoral and lumbar localisation is revealed. For the first time features of painful patterns depending at the stages is degenerate-dystrophic changes in a coxofemoral joint and lumbar department of a backbone are studied. Diagnostic and therapeutic value of medical-diagnostic blockade with local anesthetics at patients with HSS is defined.The received results of research have helped to understand features of a painful syndrome at HSS. The offered technique doppler ultrasonography vessels, feed with blood a coxofemoral joint, has allowed to diagnose infringements regional blood circulations at a hip-spine syndrome.Thanks to topografo-anatomic research are modified known and new ways therapeutic and diagnostic block, allowed to expand possibility of diagnostic search are developed.Developed algorithmic diagnostic procedure of consecutive performance of medical-diagnostic blockade with local anesthetics has allowed to raise quality of diagnostics and a choice of the optimum program of treatment of patients with HSS.
