The dissertation is devoted to optimization of diagnostics and treatment of diabetic nephropathy on a basis of monocyte functional properties, urokinase and urokinase inhibitor - PAI-1 values investigation in peripheral blood of patients with different stages of disease. The important role of monocyte, urokinase and its inhibitor - PAI-1 in the development and progression of diabetic nephropathy has been proved in this work. Our results showed that the development of diabetic nephropathy is accompanied by significant changes of monocyte migration and secretion, which are depended from the stage of the disease. Disturbance of urokinase and PAI-1 balance in blood plasma of patients with diabetic nephropathy has been revealed, which may play a potential role in glomerulosclerosis and kidney interstitial fibrosis development. Unlike the previous researches, the close association has been established between their concentration and stage of the disease. Pharmacological blockade of renin-angiotensin system in patients with diabetic nephropathy led not only to normalization of arterial pressure and improvement of kidney function, but also to decrease of proinflammatory cytokine and PAI-1 blood plasma concentrations.
