Essay

Object: percutaneus denervation a facet joints. The purpose: to raise the efficiency of treatment of lumbar spondyloarthrosis relapses due to percutaneus denervation. To establish the reasons of spondyloarthralgia relapses, to cary out its prophylaxis and purposeful treatment. Methods: clinical, neurologic, X-ray measurements, computer tomography, MRI, experimental modelling, hystomorphological research, statistic. For the first time in an experimental research with modeling of various variants of electrothermal and krio destruction of nerve we demonstrated the course of destructive, reparative and regenerative processes in various terms. We've established: the frequency of neuromas formation in different terms after nerve destruction with the help of electrocoagulation or criodestruction the frequency of akson regeneration with its growing through connective seam, we've revealed the frequency and degree of motor and censetive reinnervation of laboratory animals tissues. For the first time we studied the dynamic of main intensity at different terms after lumbar faset joints denervation and connection of perameters with follow-up results ( 6 years and later) after treatment; the freguency and reasons of spondgloarthralgia relapses. In prospective clinical and roenlgenological investihation we have shown thet usage of suggestion for prophylaxis, diagnostics and treatment of relabses could significantly raise the efficiency of lumber facet joints denervation. The usage of sensitive electrostimulation method in order to determine the possible distance from an active tip of a needle-electrode to nerve brunch of the joint can help us to carry out prophylaxis of spondgloarthralgia relapses. The usage of new technologies for diagnostic and treatment of spondgloarthralgia relapses could raise the efficiency of treatment. New methods were introduced in clinical practice of Sytenko Ins.
