In the thesis the prognostic meaning of anamnestic, constitutional-biological factors and peculiarities of ante-, postnatal period as well as cytochemical and metabolic peculiarities of cellular bioenergetics formation in mitochondria level systematically correlated with clinical syndromological peculiarities of pneumonia have been determined. The indicators for screening diagnosis of the state of bioenergy exchange (the amount of mitochondrial enzymes, total carnitin, lactate, pyruvate, and their ratio) and their significance have been discovered with the purpose of individualizing therapeutic diagnostic tactics at complicated pneumonia in children of the first year of life. Multi-level system of clinical monitoring and early diagnosis of the morphofunctional state of bioenergy exchange has been worked out. It has been proven that the risk factors (FPI, gestosis, TORCH-infections, complicated course of labor, pre-term birth, primary reanimation in the maternity room, oxygen-therapy, artificial ventilation in the neonatal period, accompanying lesions of the CNS and bronchopulmonary system) correlate with the background inhibition of the enzyme-metabolic link of mitochondrial bioenergetics and activation of anaerobic oxidation processes. The most informative clinical indicators and laboratory markers of screening diagnosis of polysystemic disorders of energy exchange at complicated pneumonia course have been defined.
