Prostate specific antigen (PSA) production in serum and ejaculate was within the age normal range. Study of cellular and humoral chain of immunity showed the suppression of T-cellular immunity, increased CD8+-lymphocytes, reduction of immune regulatory index, activation of neutrophil phagocytosis, increased concentration of circulating immune complexes. Analysis of local immunity in patients showed increased IgA, IgG, sIgA, IL-6, IL-8, TNF-alfa in ejaculate and for the first time showed that their levels don't depend on the expression of inflammatory reaction. The stimulating influence of cytokines on sperm apoptosis was analyzed for the first time. More obvious changes in all indexes are typical for CPPS ІІІA. We proposed to determine the levels of IgA, IgG, sIgA, IL-6, IL-8 and TNF-alfa in ejaculate for CAP/CPPS diagnosis and prognosis of the disease. Research of fertile function of ejaculate in patients with CAP/CPPS is recommended to supplement with determination of pro-apoptotic marker CD95+ in sperm along with determination of IL-6, TNF-alfa levels.
