Dissertation is devoted to an urgent question of advancing of complex treatment of chronic posttraumatic osteomyelitis of long bones for treatment outcomes improvement due to addition to surgical treatment of reasonable pathogenetic based complex of medicamental and physiotherapeutic methods of influence on course of an inflammation together with usage of surgical techniques. The obtaining of satisfactory outcomes of treatment of patients with posttraumatic osteomyelitis demands a comprehensive approach with rational usage of surgical and conservative methods of influence at the layer of macro and microorganism. Despite surgical treatment which decides the problem of removal of morphological substratum of purulent process and recovery of a function segment and of the limb in general, the complex of conservative measures for correction of disorders which developed in organism is necessary. Major of means of conservative treatment arc antibiotic therapy, detoxification therapy, correction of immunodeficiency, protection of vessels and protection of an endothelium on a background of adequate nutritious support. Also the correction of local disorders is necessary. The data, obtained in research, concerning changes which one arise in a segment affected by osteomyelitis established biochemical and immunological violations, which develop in postoperative period in organism, as well as changes which occur in wound after operation have allowed us to justify and to introduce in clinical practice pathogenetic based complex of conservative therapy which reduces quantity of purulent complications in the postoperative term and to improve outcomes of treatment. The grade of the short-term treatment outcomes in patients with posttraumatic osteomyelitis in the postoperative period evidence the high performance of an proposed complex of conservative therapy (2,9+0,23 - base and 1,9+0,12 grade - in monitoring groups of patients, p < 0,05). The increase in fraction of patients that are engaged in labour activity to 83,4 % in the basic group while 71,4 % in control group confirms positive and long-term effect of the proposed medical complex.
