The dissertation is dedicated to optimization in diagnosis and prognosis of combined pathology - silicosis accompanied by arterial hypertension (AH), basing on determination of hazards, common disease development mechanisms, assessment of the role of endothelin-1 (ET-1) and fibronectin (FN) as markers of endothelial disfunction, fibrosis and inflammatory process. It was stated that main risk agents for development of silicosis in foundry workers from machine-building plants in Kharkiv include: working experience under the influence of fibrogenous dust of more than 15 years, age exceeding 50 years and tabacco smoking. More expressed clinical manifestation and affection of respiratory system in patients with silicosis on the background of AH were found. It was proved that an increase of ET-1 and FN is typical in silicosis. The inverse relationship between concentration of endothelial dysfunction markers and level of functional respiratory disturbances there was stated. The presence of accompanying AH in patients with silicosis is characterized by more expressed elevation of ET-1 and FN. The direct correlation between ET-1 and FN in silicosis was revealed, particularly in the case of its combination with AH. The progression of silicosis is accompanied by more expressed clinical manifestation, endothelial dysfunction, especially in case of accompanying AH presence.
