Shop I. The state of platelet adrenoreceptor complex and leptinemia at hypertensive patients with obesity.

The dissertation is dedicated to the problem of optimization of diagnostics of metabolic disorders and their correction in hypertensive patients with obesity. The study of the issue is directed towards the research of SAS on the level of platelet adrenoreceptor complex and secondary cellular messenger activity, also taking into account the characteristics of leptin and insulin secretion and changes in lipids and carbohydrate metabolism, as well as definition of the role of those factors in the forming of myocardium remodeling. The properties of molecular mechanisms of platelet adrenoreceptor reactivity at hypertensive patients with obesity depending on the body mass index level and different types of LV remodeling are studied. It is proven that development of the insulin resistance and hyperleptinemia takes place not only in hypertensive patients with accompanying obesity, but also in hypertensive patients with normal weight. It has been possible to establish relationship the characteristics of platelet adrenoreceptors and the following characteristics: leptin level, insulin resistance state, anthropometrical characteristics, indicators lipid profile, blood pressure level and the rate of revealing the metabolic syndrome (МS) in hypertensive patients. The complex deviation the leptin production disorder, changes in a carbohydrate homeostasis, adrenergic system function and presence of obesity contribute to the clinical course of hypertension and the development of LV myocardium hypertrophy. It is proved that monotherapy iACE influences pathogenetic mechanisms of development arterial hypertension with obesity, namely positively influences sensitivity of platelet adrenoreceptors, authentically reduces leptin, insulin levels.
