Vahnenko A. Peculiarities of the clinical course and treatment of ulcer disease in combination with diabetes mellitus

The thesis is devoted to studying the peculiarities of pathogenesis, clinical course and treatment of ulcer disease in combination with diabetes mellitus according to clinical and experimental research. For the experimental gastric ulcer in combination with diabetes mellitus strengthening of depolymerization mucous protective proteins, proteinase inhibitory imbalance is observed on the background of expressed metabolic intoxication. The positive influence of the combination of pantoprazole and rebamipid on the mechanisms of pathogenesis leading to ulceration is proved. Patients with ulcer disease associated with HP, in combination with diabetes mellitus of the 2 type have maximum degree of metabolic and histomorphological changes in the gastric mucous barrier zone, accompanied by the increase of free-radical oxidation activity, endotoxemia, an imbalance of cytokine profile, morphofunctional disorders. The combination of rebamipid and antihelicobacter therapy increases the eradication rate and clinical and endoscopic remission in HP-positive ulcer diseases patients with concomitant diabetes mellitus of the 2 type. Efficacy of actovegin prescription on the background of pantoprazole in HP-negative patients with ulcer disease in combination with diabetes mellitus of the 2 type was proved.
