PhD thesis in specialty 14.01.04 - cardiovascular surgery. - Government facility "National M. M. Amosov Institute of Cardiovascular Surgery" NAMS of Ukraine, Kyiv, 2010. This thesis is devoted to the investigation of the complex problem of medical assistance to children with TGA from far-away regions at the pre- and postoperational stages, and development of the protocols of transportation, stabilization of the patient's condition, and postoperational medical assistance. The investigation is conducted on the basis of 74 patients with various anatomic types of TGA who were diagnosed, transported to cardiosurgical centres and operated on over the last ten years. It is found that in case of the confirmation of the diagnosis of TGA the patient has to be transported to the regional hospital for conducting of the stabilization measures despite the effectiveness of the measures provided in the local intensive care units. The protocols of preoperational diagnostics of the patiеnts with TGA are supplemented. The algorithm of stabilization of their condition in regional hospitals and during the durable transportation periods to cardiosurgical centers is established, the dose of infusion of prostoglandine Е1 is optimized. The EchoCG of the heart and great arteries characteristics in patients with TGA in periods remote from the time when various types of surgical corrections is investigated. The protocols of postoperational EchoCG monitoring and medical assistance are proposed.
