The thesis presents a comprehensive analysis of clinical and radiological, biomechanical, microbiological, immunological studies and surgical treatment of 101 patients with post-traumatic suppurative arthritis ankle. The dissertation is devoted to the diagnosis and treatment of purulent arthritis postravmatychnoho ankle. In this work the generalized and systematic analysis of causes and factors of purulent process. In all patients selected microorganisms in the culture of 94,6%. This monoculture was found in 37.7%. Vid 62,3% of patients simultaneously or in dynamic allocation of observations from 2 to 4 species of microorganisms. By kinds of microorganisms dominated cultures allocation patients with staphylococcus (in 77,3%). P. aeruginosa, which is probative indicator of the presence ekzoinfikuvannya was highlighted as a whole, from 37.7% of patients. When analyzing the sensitivity of selected microorganisms to antimicrobials determined that for the last 5 years, frequency allocation metytsylinrezystentnoho S. aureus increased from 7,0% to 50,0%. Revealed that the main defense against infectious pathogens of purulent-inflammatory process in the studied group of patients is due to humoral part of immunity, as evidenced by the increase of immunoglobulin classes A, M and G against the background of high levels of circulating immune complexes. As a result of biomechanical examinations of patients with ankle joint ankilozovanym The optimum position for walking foot when performing arthrodesis. The algorithm of surgical treatment of posttraumatic purulent arthritis of the ankle, which provides a differentiated approach to selecting the optimal method of surgical intervention to achieve elimination of purulent-inflammatory process of restoration of limb function in a short time after injury.
