Essay

The aim of this dissertation is to describe peculiarities of hormonal-metabolic and functional disorders of patients with type 2 diabetes mellitus in order to improve early diagnosis of diabetic cardiomyopathy. Subclinical phase characterized by increased level of endothelin-1. Functional phase - by the IV type of dyslipoproteinemia; increase in insulin concentration, leptin, C-peptide, hs-CRP, TNF-a, PECAM-1, TG, indices HOMA-IR and decreasing of QUICKI-index; increase of the systolic and diastolic blood pressure variability parameters, formation of disproportional daily rhythm of blood pressure. Functional-organic phase is characterized by the IIb type of dyslipoproteinemia, significant increase of atherogenic coefficient, levels of myoglobin, interleukin-8 and endothelin-1; may be accompanied by hypertension with left ventricular hypertrophy; reduction of sympathetic, parasympathetic and humoral links tonus with predominance of sympathetic link of vegetative nervous system, by formation of steady-state circadian profile of heart beats frequency and sympatho-parasympathetic coefficient.
