Essay

Subject of the study: diabetic nephropathy (DN). Objective of the study: to analyze lipid metabolism features, apolipoprotein and leptin contents of patients that have diabetes mellitus (DM) with DN at different stages; to suggest methods of corrections discovered disorders. Methods of the study and devises: clinical, instrumental, biochemical, immunoenzyme, immunoturbidimetric, statistical; autoanalyzers Сobas Mira plus, Cobas Mire S, Hitachi 911, Stat-Fax-303+ (Czech, Sweden, USA). Theoretical and practical results: certain features of changes of indexes of lipid exchange - lipids, apolipoproteins and leptin at different stages of DN, their function in pathogenesis of nephropathy of patients with DM of the 1st and 2nd types. Increasing of apolipoprotein B level and decreasing of apolipoprotein A1 level in blood of patients with DM of the 2nd type with albuminuria is established. Idea of possibility of microalbuminuria decreasing with help of hypolipidemic therapy of patients with DN is been extended. Assessment of lipidic and apolipoprotein metabolism state, content of leptin can be additional method of diagnostics and prediction of DN course. Novelty: for the first time the increased level of leptin in patients' blood with DM of 1st type with DN presence has been discovered, including pre-clinical stage. The possibility of decreasing of protein excretion with urine with help of prescription of hypolipidemic agent symvastatin has been proved. Implementation stage: the results of study are used in clinical practice of nephrology clinic with wards for endocrinological patients of Head military medical clinical centre - "GVKG". Application area: endocrinology, nephrology.
