Dissertational research is devoted to development and to experimental studying of an opportunity clinical use synthetic Ukrainian biodestructive implant "Оcoform" at carrying out of the modified not punching deep sclerectomy at patients with a primary open angle glaucoma with the raised proliferative activity and high risk postoperative cicatrization in a zone of a surgical intervention. Diagnostic efficiency offered screening-test of tear on markers of risk of a superfluous proliferation at patients with a primary open angle glaucoma makes 73 % that enables to prognosticate current of the postoperative period and to select tactics of surgical treatment. The offered operation gives proof hypotensive effect during all time of observation, and in the remote period shows stabilization of hydrodynamical parameters in 83,3 % of the operated patients with the high risk postoperative cicatrization in a zone of a surgical intervention (33,3 % without additional hypotensive therapy). Proof normalization of an intraocular tension has allowed to stabilize and improve clinically visual functions (visual acuity in 73 % and field of vision on 83 %) in the late postoperative period that allows to recommend it for surgical treatment of patients with primary open angle glaucoma and high risk of a cicatrization in zone of a filtration (according to screening-test: levels free arachidonic acid in tear > 10,0 pmol/mg protein, ureas > 20,0 nmol/mg protein).
