Dissertation is devoted to determination of the role of inflammatory cytokines - tumor necrosis factor ? (TNF-?) and interleukin 6 (IL-6) - and homocysteine (HC) in patients with ischemic heart disease (IHD) combined with diabetes mellitus type 2 (DM 2), and studying the effects of benfotiamine on their correction in order to improve the efficiency of therapy of patiens with IHD and DM 2. It was found that patients with IHD and DM 2 have significant hyperhomocysteinemia, and increasing of the HC rate depends upon dyslipoproteinemia, obesity and insulin resistance. TNF-? and IL-6 in circulation of studied patients considerably exceeded the indexes of control group, negative correlation took place between TNF-?, IL-6 and functional indexes of the left heart ventricle. It is determined that dyslipoproteinemia and insulin resistance leads to increasing of IL-6 in patients with IHD and DM2. A positive influence of benfotiamine and pyridoxine combination on TNF-?, IL-6 and HC levels both in men and women is proved. At the same time the complex therapy that includes benfotiamine and pyridoxin leads to positive changes in diastolic indexes of left heart ventricle.effectiveness of the proposed treatment was proven including excessive activity of metalloproteinase.
