Essay

Object of the study: papillary thyroid carcinoma. Aim of the study: to assess the remote results of treatment (survival, lethality, relapses) and psychosocial aspects of quality of life of patients with papillary thyroid cancer. Methods of study and equipment: clinical, instrumental, surgical methods, survey, mathematic and statistical methods; ultrasound device ‘Toshiba’, ‘Leica’ microscope. Theoretical and practical results: an analysis of the treatment of papillary thyroid carcninoma in a patients’ cohort who were children and adolescents at the time of the Chernobyl accident indicate favourable treatment outcomes. Practical guidelines have been developed, on the tactics and volume of surgery for patients with papillary thyroid carcinoma depending on process spreading in relapses and in case of primary non-radical interventions. The necessity has been proved, of developing programmes of psychological rehabilitation of patients after surgery for a papillary thyroid carcinoma. Novelty: For the first time, results have been obtained, of a detailed analysis of papillary thyroid carcinomas based on data of 5-year, 10-year, and 15-year follow-up of a cohort of patients who were children and adolescents at the time of the Chernobyl accident. A prognostic analysis has been conducted and the impact has been determined, of main risk factors on the results of surgical treatment of thyroid cancer following the Chernobyl accident, occurred in patients with increased risk (aged 0 to 18 years in April 1986). It has been established that the number of relapse cases is higher in case of organ-saving operations, as well as in the presence of metastases to cervical lymph nodes and mediastinum. For the first time, individual indices of health have been assessed, medical indices of aspects of life quality in patients having been operated for a papillary thyroid cancer. Degree of introduction: based on the results of a study of psychological rehabilitation of patients with malignant thyroid tumours after combined treatment.
