Essay

Object: bone healing and the restoration of the lower limb function in injured children and adolescents with diaphyseal femoral fractures. Propose: To improve results of the treatment of the diaphyseal femoral fractures in children and adolescents by development and grounding of the functional method of treatment and external pin device. Methods: clinical, radiological, mathematical and physical. New knowledge about stress-strain distribution in the system "femoral bone fragments-external fixation device" was obtained. It is founded that under stress system works in an elastic mode with linear displacement of the bone fragments in a relation to one another by 15 mm. Elastic properties of the system depend on the street in the regeneration zone. More elastic strain means more stress in the regeneration zone. This situation defines shape of the preinstall regenerate. It is founded trend in the stress magnitude in the "pin-bone" joint. This allows recommending functional loading of the injured limb. We found zones of the highest stresses in the system "fragments-device" and the most affected joints. This concern allowed us to do modification of the device and the method of its using. On the basis of theoretic, experimental, clinical and radiological studies methods of the diaphyseal femoral fractures in children and adolescent has been developed. It is improved external pin device (patent of Ukraine № 9372) and developed method of its use. This give an opportunity to propose this technical development into serial manufacturing and the developed method could be introduced into the practice of orthopedics and traumatological departments of the hospitals. This method has been introduced to the clinical practice of municipal institution of minister of health "Kharkov regional traumatological hospital", pediatric traumatological department of municipal institution of minister of health "Clinical multidisciplinary hospital N17 of the factory named by Mulish of Kharkov city", traumatological department of Chugged regional hospital of the Kharkov region, City hospital of Kremenchug in Poltava region. Main statement of this study is used in the educational process in the cathedral of Traumatology and Orthopedics of Kharkov Academy of Postgraduate education. Traumatology and orthopaedy.
