Essay

The results of surgical treatment of 211 patients with intracerebral hematomas (ICH) and inraventricular hemorrhages (IVH) because of aneurysms rupture of the anterior half-ring of arterial circle of the brain, have been treated in SI "Institute of neurosurgery named after acad. A.P. Romodanov of Academy of Medical Sciences of Ukraine" from 2006 to 2010 were analyzed. There were 127 (60.2%) men and 84 (39.8%) women. Age of patients ranged from 12 to 72 years, in average 46.6 years.Depending on hemorrhage anatomical variant patients were divided into 2 groups:1) ICH - 136 (64.5%) cases, 2) IVH - 75 (35.5%) cases: 56 (74.7%) patients had IVH due to ICH breakthrough into ventricles, 19 (25.3%) -had only IVH. On admission state of 143 (68%) patients corresponded to III-V degree of severity according to Hunt-Hess scale. 21% patients with ICH and 40% - with IVH had repeated hemorrhages. At ICH patients' state severity depended on hematoma volume and localization, brain structures dislocation; at IVH and ICH combined with IVH - on degree of blood into the ventricles.Hematoma localization depending on aneurysm bottom direction was studied. The efficiency of basal and ventricular fibrinolysis in patients with massive subarachnoid hemorrhage and IVH was studied.Due to differentiated treatment tactics the lethality was decreased from 30-41% in 2006-2007 to 17-18% in 2009-2010. The number of good and satisfactory treatment results respectively increased from 31-54% to 60-68%.
