Essay

This study of clinical pathogenetic, neuromorphological, neurophysiological and pathochemical features of dementia in a cohort of elderly women supports the importance of combination of Alzheimer's disease and vascular dementia. The study has demonstrated importance of chronic inflammatory process and key factors contributing to dementia progression include insulin resistance in Alzheimer's pathogenesis. The study was able to distinctly demonstrate positive influence of pioglitazone - agonist of peroxisome proliferator-activating receptors gamma (PPAR?) on improved cognition, neurophysiologic functions and manifest decline of inflammation and insulin resistance markers, especially for persons with the presence of 12Ala allele of PPAR? gene. The study allowed to develop and introduce a new treatment methodology for persons with insulin resistance diagnosed with Alzheimer's Type I using pioglitazone in clinical practice.
