Essay

This thesis covers development and implementation of the new methods of examination and treatment of patients with odontogenic inflammatory diseases, cyst of the maxillary sinus, and more effective and minimally invasive endoscopic surgery in the affected sinuses. Typical radical surgery on the maxillary sinus on patients with inflammatory processes is extremely traumatic and may result in postoperative complications. Alternative ways are the methods we developed for surgical treatment of odontogenic maxillary sinusitis through the front wall of the sinus and over a causal tooth: vestibular endoscopic maxillary sinustomy in the absence of a causal tooth (VEMS-1) and vestibular endoscopic maxillary sinustomy preserving causal tooth (VEMS -2). The results of implementation of both VEMS methods evidence that they are considerably less traumatic as compared to traditional maxillary sinusotomy and help to achieve positive results in 98 % operations. Implementation of our VEMS surgical technique made it possible to reveal its advantages over the traditional methods of radical maxillary sinustomy, to conduct visual exploration of the operated sinus, to implement sparing intra-cavity surgery, realize sanation of odontogenic niduses of inflammation, to determine the state of the natural fistula of maxillary sinus, to reconstruct the oro-antral fistula, to save the causal teeth, to avoid damaging the intact tooth, to reduce trauma from surgery, to reduce operation time and inpatient treatment period.
