Essay

The dissertation is devoted to actual problems of endocrinology - the diagnosis of autoimmune Diabetes Mellitus type 1 and autoimmune thyroiditis and their combination in the case of one patient. For this purpose 2144 inhabitants of the industrial city of Mariupol were surveyed. Screening of the function of thyroid gland, glucose metabolism and available markers of autoimmune thyroid lesions and autoimmune lesions of ?-cells of the pancreas were used. Screening was conducted among three groups of patients: patients with Diabetes Mellitus type 1, patients with AIT and healthy persons (control group). Screening enabled to detect from 3,2 % to 16,6 % new-onset cases of Diabetes Mellitus type 1, thyroid hypofunction was determined in 10 % of patients with Diabetes Mellitus type 1 and in 10,8 % of persons in the control group. The combination of autoimmune thyroiditis and Diabetes Mellitus type 1 in one patient was detected in 0,03 % (142) of patients. According to statistical reports the ratio of prevalence of registered cases of the autoimmune thyroiditis, Diabetes Mellitus type 1 and its combination is about 16:6:1. At the same time, the frequency of detection in the control group of available markers like TPO-ab was 45,8 %, GADA-ab - 25,3 % and the rate of simultaneous presence of these markers in one person was 12,9 %. Thereby in our case the ratio of elevated level of available markers was 3 : 2 : 1. For a correct analysis of the prevalence of Diabetes Mellitus type 1 and autoimmune thyroid disease in the everyday work of public health and medical statistics should be used only the right terms of the International statistical classification of diseases and related health problems. Our work proved that the true prevalence of Diabetes Mellitus type 1 and autoimmune thyroid diseases could determined only on the basis of active population-based surveys, especially in groups with risk factors.
