The dissertation is devoted to the matters of tibia bones diaphyseal fractures treatment, grounding and development differentiated approach and perfection of LIOS technology, aiming at the results of treatment improvement, as well as at shortening the period of limitation the injured person's physical ability. The research shows that the absence of fixation the fibula at low fractures of shin bones resulted in 38.11 percents primary invalidity of all the cases. Biomechanical investigation has defined, that the osteosynthesis of both shin bones had not caused any reasonable changes in the tension of the bones, compared to the separate osteosynthesis of tibia bone in all the variants of lower extremity thrusts. However, it is very important that additional fixation of fibula leads to less movements of its fragments and allows to decrease reasonably the tension of tibia fixator. In case of unstable tibia fractures, fibula has to be fixated not only in the middle, but also in the lower third. In case of stable fracture, it is enough to fix only lower third of a fibula. The research analyses the treatment results of 219 patients suffering from diaphyseal tibia fractures. The main group consisted of 103 patients suffering from diaphyseal tibia bone fractures, who undergone surgical treatment using the locking intramedullary osteosynthesis. The control group consisted of 116 patients suffering from diaphyseal tibia fractures, who undergone extramedullary and extrafocal osteosynthesis.
