Essay

Object: acute anteromedial instability of knee joint. Aim: to improve the treatment results of patients with acute anteromedial instability of knee joint by perfection of diagnostics and development of new surgical methods for repair of capsule and ligaments. Methods: clinical, X-ray, ultrasonic diagnostics, MR-image, experimental, mathematical modeling using method finite elements, statistical. During theoretical and mathematical modeling using the method of interpolation by splines new mathematical model of capsule and ligaments apparatus of knee joint was created. The model proved importance of different ligamentous structures for knee join stabilization. At first during MR-tomography injury signs of posterior oblique ligament of knee joint was determined. Firstly the method of preoperative planning for surgical treatment of patients with injuries of ligamentous apparatus of medial knee join was developed and introduced in clinical practice (patent UA № 61768 А). During experimental investigation at cadavers new data about spatial relations of fixing points of capsular and ligamentous structures of medial part of knee joint. That allowed develop of new mini-invasive surgical method for repair of acute injuries of anteromedial part of knee joint using arthroscopy. Developed scheme of diagnostics of injuries of capsular and ligamentous structures of anteromedial part of knee joint permits increase the accuracy of injuries diagnostics and identify optimal surgical approach. Proposed algorithm of surgical treatment of patients with injuries of capsular and ligamentous apparatus of anteromedial part of knee allows choose true treatment approach subject to mode and degree of instability. The use of developed mini-invasive surgical method for repair of acute injuries of anteromedial part of knee joint using arthroscopy permitted decrease traumatic injury and improve the results of surgical treatment. Results was worked into the clinical practice at UC "Zaporizka regional clinical hospital", MSCh "Моtor Sich", UC "Luganska regional clinical hospital", UC "Dnipropetrovsk regional clinical hospital named І.І.Mechnikov", UC "Odessa regional clinical hospital", UC "Volyn regional clinical hospital", public enterprise "Sytenko Institute of Spine an Joint Pathology, Ukrainian academy of medical sciences".
