Essay

The aim of the study was to increase the effectiveness and safety of regional anesthesia for operations on the lower extremities by optimizing of nerve visualization, verification of needle introducing and the local anesthetic distribution. Based on surveys of representative groups of volunteers, patients, who had undergone regional anesthesia, and an experimental series the theoretical generalizations were performed in the thesis as well the new important scientific problem was solved - the method of visualization of peripheral nervous system was first proposed, including sonography of lower limb nerves for regional anesthesia guidance based on the individual structural features and location of nerves. Three-dimensional vector modeling based on segmentation of sonographic data revealed specific features of nerves. Ultrasound control of regional anesthesia allows to gain for higher quality scores of regional anesthesia than neurostimulator control. Combined use of ultrasound and neurostimulator leads to significantly less unsuccessful blocks and need to needle transposition during manipulation. The algorithm of regional anesthesia was proposed.
