Study is grounded on results of screening by aldosteron/renin ratio among 578 outpatient and 542 hospital patients with 2nd stage arterial hypertension followed by standardized investigation of 128 patients with PHA during 2009-2011 years. Additionally 151 patients with PHA treated surgically were studied among total group of 570 adrenalectomies on different causes within 1996-2011 years. High prevalence of PHA (10-13 %) among selected hypertensive patients was shown. Appropriateness of proposed step-by-step algorithm of diagnosis, differentiation and choosing of treatment options, necessity of selective use of adrenal vein sampling were proven. Results of PHA treatment with aldosteron receptors antagonists were analyzed with clear evidence of its efficacy. Complex approach to diagnosis and justification of indications to surgery led to 3 times decreasing of relapse rate after adrenalectomy (from 46 % to 16 %).
