Essay

The thesis focuses on the actual problem of modern neonatology – study of pathogenetic role of ferrokinetics violation in formation of brain pathology in premature infants. The risk factors of hypoxic-ischemic brain damage in premature infants were investigated, the role of iron metabolism indicators in pathogenic mechanisms and their dependence on gestational age were studied. It was proved that the most significant ferrokinetics violations had premature infants with intraventricular hemorrhages, periventricular leukomalacia and gestational age less than 32 weeks. Connections between the level of indicators of iron metabolism and severity of brain damage in premature infants were established. It was shown, that in the process of intraventricular hemorrhage accumulation of iron in brain tissue occurs, which plays an important role in formation of white matter damage - in 75 % of deceased children perifocal centers of leukomalacia were identified. The levels of BDNF in infants with periventricular leukomalacia and interaventricular hemorrhage in dependence with gestational age were defined. The data obtained allowed to complete the scheme of pathogenesis of hypoxic-ischemic damage of central nervous system in premature infants by including ferrokinetics violations as one of the links of adverse affect on brain damage formation in premature infants.
