Object: effects of tumor necrosis factor, expressed gelatinous nucleus, the nerves and perineural tissue structural changes in them under the influence of drugs of biological therapy. Objective: To examine the influence of drugs, inhibitors of tumor necrosis factor alpha in the conservative treatment of herniated discs animals and humans. Methods: The morphology of electron microscopy and morphometry to study the structure of nerve and surrounding tissues; Experimental animal modeling to create a model, clinical and radiological using magnetic resonance and computed tomography for the diagnosis and evaluation of conservative treatment of herniated discs in patients osteochondrosis. The study represents a new direction in the study of mechanisms of inflammation in the nerve and perineural tissue under his contact with disc herniation. In the experimental modeling herniated disc set that transplantation gelatinous nucleus (as part of the herniated disc) on the nerve leads to pronounced inflammation and destructive changes in its structure and perineural tissues. This inflammation and swelling, accompanied by demyelination of nerve fibers, damage to axons observed in the early postoperative period and are of lasting nature. The disturbances of the shape variability diameter myelin, bezmiyelinovyh nerve fibers and myelin destruction layer. Destructive changes such as thinning and lysis neyrofilamenty recorded in bezmiyelinovyh fibers. For the first time in experiments on animals conducted correction inflammatory and destructive disorders of the nervous tissue, caused by the action of cytokines expressing jelly nucleus implanted in the area of nerve agents by using biological therapy - adalimumaba and infliksimaba. It is shown that under the action of these drugs reduces swelling, inflammation in the nerve and perineural tissues, which contributes to the delay destruction of nerve tissue. However, destructive changes in peripheral nerves before treatment on the background effects of tumor necrosis factor-alpha after gelatinous nucleus of rats on a nerve, though there have been, but were significantly reduced compared with the control group animals. Experimental research is the basis for the use of the developed model to study various options for biological therapy in the conservative treatment of herniated disc, aimed at inhibiting the biosynthesis of tumor necrosis factor alpha and other proinflammatory cytokines.
