Research object: primary open-angle glaucoma. In the given study the author researched the development of postoperative complications in patients with primary open-angle glaucoma - excessive scarring in the area of operative intervention and postoperative astigmatism after antiglaucomatous operations depending on the conditions of general and local immunity - indices of phagocytosis, autosensebilization of the organism to the cerebral cortex and hypothalamus, the level of cytokines (TNF а, IL 1 b, IL 4, IL 6) and cortisol both in the blood and in aqueous humour. More scientific data as to the condition of local immunity of organ of vision in primary glaucoma are given. The correlation of high degree between the level of cytokines in aqueous humour and the development of excessive scarring in the area of operative intervention, and postoperative corneal astigmatism after antiglaucomatous operations was established. The improved technology of sinusotrabeculectomy was developed and put into practice that allows to reduce the rate of corneal astigmatism. A new method of prophylaxis of postoperative development of corneal astigmatism was developed and put into practice. The results of the work are adopted in 7 leading ophthalmic hospitals of Ukraine, the main provisions of the work are included in lecture's program and practical work of ophthalmology chairs in 4 Medical Universities of Ukraine.
