The thesis is devoted to the problem of improving the diagnosis and effective treatment of heart failure ischemic patient with left ventricular systolic dysfunction based on the study of clinical features, immune-inflammatory markers, quality of life and cardiohemodynamics, parameters of carbohydrate and lipid metabolism and their dynamics on the background of standard treatment with bisoprolol use depending on the ?1-adrenergic receptors gene polymorphisms. The prevalence of Ser49Gly and Arg389Gly polymorphisms in diabetic patient with heart failure and clinical features of the disease in patients with diabetes mellitus and without on the background of standard therapy with bisoprolol use depending on the ?1-adrenergic gene polymorphisms were determined. The standard therapy with bisoprolol within 12 month improves the clinical status, increase the exercise tolerance regardless of Ser49Gly ?1-adrenergic gene polymorphisms. More favorable effects in clinical symptoms of heart failure reduction, cardiohemodynamic parameters improvement were observed in Arg389Arg and Arg389Gly genotypes carriers in contrast to the Gly389Gly ?1-adrenergic gene polymorphisms carriers.
