Zozulya Y. Surgical treatment of the spastic hand.

The thesis is devoted to study of different methods of treatment of the spastic hand of different genesis and is based on results of inspection and treatment of 97 patients with spastic hand, been operated or treated conservatively in the period from 1989 to 2011.Patients' state was evaluated according to scales of strength and spasticity by Ashworth, spasticity type - by House, hand's function - by Usoltseva classification. Before and after operation electromyography of the damaged extremity and video-registration of movements range were carried out.Best results foe certain were obtained after selective fasciculotomy supplemented by radial nerve temporary neurostimulation.The spastic hand function effectiveness for certain depends on spasticity initial level by Ashworth, hand's initial strength by 6-mark scale, difference of these two factors numerical values, hand's spasticity type by House, it's initial function, patient's mental state, extend of spasticity. The duration of period without relapse for certain depends on selected method of treatment, spasticity level by Ashworth and hand's strength by 6-mark scale before and after treatment, hand's function, spasticity type by House, extend of nervous system damage, patient's sex.
