Essay

The thesis is devoted to study of clinical signs, flow, diagnostics features, close and follow-up consequences of diffuse axonal brain injury (DAI) in patients of different age groups. The research is based on results of complex investigation and treatment of 107 injured persons with DAI.As a rule DAI is revealed in young patients, also in children and elderly patients (in 82.3% cases), mostly it is caused by road accidents and catatraumas (88.8% cases). Coma is a particular state after trauma, frequency of stem symptoms in acute period of DAI is high. There are two variants for patients' exit from coma: by "switch-on" type and transition to vegetative state.In process of brain changes expressiveness increasing according to CT coma deepness and duration as well as frequency of vegetative state and lethality increased in all groups of patients. Brain atrophy is a particular sign of DAI, it's probability increases with patient's age.DAI close consequences are characterized by high frequency of patients' deep disability. Frequency of negative consequences increases with patients' age, in case of late hospitalization in specialized unit, at severe extracranial pathology. Follow-up consequences of DAI in patients younger than 18 years and 19-29 years usually are characterized by good recovery and moderate disability, in patients aged 30-44 and 45-59 years - by deep disability.
