Essay

The object: neovascular glaucoma (NVG) of diabetic and post-thrombotic genesis. The aim: to increase the efficacy of surgical treatment of neovascular glaucoma of post-thrombotic and diabetic genesis by means of combined application of viscoelastic and episclera excision in the area of surgery. The methods: general ophthalmic; optical, ultrasound biomicroscopy (UBM). The data on pathomorphological differences in the structural changes of an eye tissue during postocclusal genesis and diabetic genesis are specified; the morphological structure of scleral uveal adhesions is described. Viscoelastics are proved to perform the barrier and drainage function in episclera excision viscocycloiridoretraction (VCIRE) surgery; the risk of connective tissue growth after episclera flap excision is reduced. VCIRE means viscoelastics intake into suprachoroidal space and episclera excision in the surgery area. The method promotes normalization of IOP with 42,8 % patients within 3 years. The patients with postthrombotic NVG proved to have IOP between 17 mmHg and 24 mmHg in 70-75% cases within 3 years. VCIRE makes the postoperative complications risk reduced to 10%. The results of the research are implemented in units №3 and №4 of L.L. Girshman Kharkiv City Clinical Hospital and "Regional Clinical Hospital- Centre of Urgent Care and Medicine of Catastrofies"(CUCMC), departments of ophthalmology at Kharkiv Medical Academy for Postgraduate Education (KMAPE) and Kharkiv National Medical University (KNMU).
