Essay

The thesis is devoted to improving the efficiency of diagnosis and treatment of hypertensive patients with concomitant type 2 diabetes mellitus by identifying the character of changes of insulin's sensitinogens and antagonists depending on level of insulin resistance and estimations of cardiovascular risk based on the clarification of the level of sCD40L. This study is demonstrated that in hypertensive patients with concomitant type 2 diabetes mellitus there were occurred significant disorders of lipid and carbohydrate metabolism, cardiohemodynamic parameters, daily monitoring of blood pressure, which influenced the forming of insulin resistance. Adipose tissue dysfunction is manifested by increased levels of resistin and by decreased levels of adiponectin. The pathogenic role of sCD40L was determined in the forming of cardiovascular complications.The possibility of therapeutic correction of metabolic disorders in a cohort of surveyed patients with using ramipril was proved.
