Essay

The thesis deals with the peculiarities of carditis course in infants on the background of congenital cytomegalovirus infection, interrelation between hemodynamic processes in the myocardium and the development of endothelial dysfunction, natriuretic peptide serum content. Their pathogenetic role in the development of cardiohemodinamic and vegetative support disorders of myocardium was determined. Parameters of intracardial hemodynamics in infants were defined. It was found that observed children had strain of sympathetic link of autonomic nervous system, manifested by decreased activity of parasympathetic and suprasegmental effects and by the relative increase in activity of the vasomotor center. Presence of diastolic dysfunction, especially type II, was accompanied by depletion of sympathetic link of autonomic nervous system and it evidenced about violation of vegetative control of cardiac activity and development of energetic and metabolic deficiency. It was shown that endothelial dysfunction that developed in children with carditis against the background of congenital cytomegalovirus infection had long course despite on normalization of clinical presentation of disease. It was proved that duration of the endothelial dysfunction is connected with the type of diastolic dysfunction. Prescription of IACE as base therapy, its expediency and duration were pathogenetically grounded.
