Object: damage to the distal tibia and ankle. Objective: to determine the damage distal tibia, justify algorithm and tactics of the minimally invasive treatment of patients. Methods: clinical, radiological and radiometric, anatomical study of the bones in the distal tibia (bone structure and location of vessels), the method of mathematical modeling of the stress-deformed state of elements of the ankle joint and related segments. New knowledge about the nature of the damage and displacement of bone fragments in case pilon fractures and the indications for selecting the method for minimally invasive treatment of patients. Selected types of damage depending on the fracture lines and the number and size of the bone fragments. An improved method traction reduction of the fracture by ligament-taxis when pilon fractures and developed options for fixing fragments by apparatus external fixation devices depending on the type of fracture. The technique radiometric assessment of congruence ankle. Defined the characteristics of the stress-strain state of the ankle joint, determined the types discongruence, clinical signs and gives their prognostic assessment. Advanced techniques of treatment of fractures pilon devices of external fixation allow you to restore the anatomical shape of the hip and the basic function of the limb. Developed a method of radiometric evaluation of the form of distal tibia allows objectively to estimate a condition and restoration of the function of the ankle joint. The devices and methods of fixation (Patent of Ukraine № 36275 and № 37693) provide a stable fixation of bone fragments, ease of surgical interventions.
